Download your requested form below. Each form is an interactive PDF that can be filled in on a desktop computer. Once filled out and saved on your computer, it can then be uploaded using the form to the right.

How do I complete the downloadable forms?
Each form is an interactive PDF with checkboxes and input fields where you may provide your answers. Click on each link to download the PDF file to your computer or mobile device (we recommend using your computer), or right-click / control + click on the link, and Save As… to save the file.
Open the file in Adobe Reader or Adobe Acrobat. If you do not have Adobe Reader installed, you may download it for free here.
Fill in the form fields where requested, and click Save. Your changes will be saved to the file, and you may then use the Upload form to send it to us.
How do I digitally sign a document?
On forms that require a digital signature, you will be asked by the Adobe program to store a digital signature on your computer. If you have not set one up before, the program will guide you through creating your first digital signature. Can I fill in the form (partially or completely) manually?
Yes! If you have access to a printer and a scanner or smartphone that can take high resolution images, you can certainly print out our PDFs and fill them in by hand if you choose.
Save the PDF to your computer, and print it. You may also opt to fill in most fields on your computer, prior to printing. After printing the document, please fill in all remaining required sections.
If you have access to a scanner, please scan the printed, completed document, save it as a PDF or high resolution JPG file, and upload it via our Upload form.
How do I submit copies of my ID or insurance cards?
You may take a clear photo of the front and back of your respective cards, and upload the photos in their respective fields.
Once you have taken your photos, email them to yourself, or otherwise download them from your cloud account to your computer. If you are using your mobile device to submit all your documents, you can just upload the photos directly.
You may also choose to create scans of the back and front of your cards using a scanner.
